Advanced Nurse Practitioner (ANP) Primary Care

We are delighted to be recruiting an Advanced Nurse Practitioner to join us one day a week on Wednesdays, supporting frailty and care home residents across the network. This is a rewarding opportunity for an experienced ANP who is passionate about improving outcomes for older adults with complex needs.

In this role, you’ll use your advanced clinical skills to deliver proactive, person‑centred care for care home residents, providing expert assessment, diagnosis, prescribing and management planning. You will work closely with GPs, pharmacists, district nurses, palliative care teams and care home staff to ensure residents receive timely, coordinated and compassionate care.
