Responsibilities

• Meet medication related needs of patients, including resolving and preventing drug‑related problems.
• Work with patients and the health‑care team to provide direct patient care.
• Participate in patient education.
• Facilitate medication reconciliation activities.
• Teach students.
• Provide education to nursing and medical staff.
• Support the drug distribution system.

Qualifications

• Current registration in good standing with the Ontario College of Pharmacists in Part A of the register.
• Practical knowledge of pharmacology, current best practice clinical guidelines, and ability to interpret lab values....
